Schedule an Appointment

Existing patients:

-To schedule by phone call 360-282-4014 (Cedar Avenue Integrative Medicine front desk)

-To schedule online, scroll down to use the calendar tool below or login to ChARM and use the ‘Request an Appointment’ feature in the ChARM portal: https://phr2.charmtracker.com/login.sas

-If I have seen you before at other locations, please follow the instructions for new patients.

New patients:

-To schedule by phone, call 360-282-4014.

-To schedule an appointment online, use the scheduling tool below. Be sure to select a new patient appointment. New patient appointments are scheduled for 1 hour but depending on your needs the appointment may not last the full scheduled time.

-Paperwork and consent forms will need to be completed in advance (available through patient portal) prior to all Telehealth appointments. For in person appointments, patients may complete paperwork through the portal or in office prior to the appointment.

-Prospective patients interested in a 15 minute meet and greet to discuss health goals and provider fit may call the front desk to schedule (360-282-4014). No medical advice given at meet and greet appointments.
